PLEASE READ CAREFULLY

Awarding of the Leo J. Monty Scholarship is at the discretion of the Leo J Monty Scholarship Committee as outlined in the SCHOLARSHIP BY-LAWS (Link).

All fields below in the STUDENT APPLICATION FORM are required for submission.

A Letter of Interest outlining your academic and extracurricular achievements must be submitted in the form. In this letter, please let us know your intended use for the scholarship money and targeted career path and course of study/college major.

In addition, a scanned copy of your high school or current college transcript (pdf file accepted) is required BEFORE STARTING this application. This will be attached and uploaded in the form below. Please make sure you have this information prior to completing the application, as the system will not allow incomplete applications.

A copy of your submission will be emailed to you acknowledging receipt.
